Acrobat form viewing problem

Solution IMPORTANT NOTE TO PREVIEW USERS (MAC):
There is a known problem of compatibility between Preview (Mac OS X's application for displaying images and PDF documents) and Adobe Acrobat. PDF forms filled out in Preview do not display all form data when opened in Acrobat. If you fill out this form using Preview, you MUST save your file as a new PDF through the Print menu. Select File -> Print -> PDF -> Save as PDF.... You may then upload the new PDF to the intranet system.

Note:
For anyone receiving a filled form with non displaying data a quick work around is to do a "save as" RTF file. This will show all filled data when the file is opened in MS Word.
